ABSTRACT
Purpose: To demonstrate, using clinical examples, the potential of SPECT/CT at different stages of follow-up in patients with bone tumor involvement treated with radical intraoperative thermal ablation.
Material and methods: The paper presents two clinical cases of patients with bone tumors who underwent radical intraoperative thermal ablation developed at the Cancer Research Institute of the Tomsk National Research Medical Center (Tomsk NRMC). During follow-up, structural changes in bone after high-temperature exposure, as well as metal-related osteosynthesis artifacts, made imaging findings equivocal. To address these limitations, the extended capabilities of SPECT/CT using 9mTc-Pyrfotech were applied. Imaging was performed 3 hours after intravenous administration of the radiopharmaceutical on a Symbia Intevo Bold hybrid SPECT/CT system (Siemens) according to standard protocols. Quantitative assessment of radiotracer uptake in pathological foci was performed using the standardized uptake value (SUV).
Results: SPECT/CT enabled interpretation of indeterminate bone changes that, based on planar bone scintigraphy and spiral computed tomography, had been regarded as post-therapeutic.
Conclusion: The presented cases indicate that incorporating SPECT/CT into the diagnostic algorithm for patients with bone changes that are difficult to differentiate provides a reliable and objective assessment. This approach may be useful not only for improving differential diagnosis between benign and malignant bone alterations, but also for evaluating treatment efficacy.
Keywords: bone tumor lesions, radical intraoperative thermal ablation, SPECT/CT
