ABSTRACT

Purpose: To present a rare clinical case of inguinal-femoral hibernoma and a brief review of the literature on the possibility of differential diagnosis with liposarcoma.

Material and methods: Patient P., 67 years old, was admitted to the clinic with complaints of pulling pain in the right inguinal region, occurring during walking, which had been bothering him for a long time. Outpatient magnetic resonance imaging (MRI) of the pelvis was performed, which revealed a space-occupying formation in the right inguinal region with a pronounced fatty component. For clarification of the diagnosis and surgical treatment, the patient was referred to the Burnazyan Federal Medical Biophysical Center. Laboratory data at admission were unremarkable. As part of further examination and preoperative planning, multispiral computed tomography (MSCT) of the chest, abdominal cavity, retroperitoneal space, and pelvis with intravenous (IV) contrast was performed. According to the study, an extragenital formation of inguinal-femoral localization on the right was determined, measuring 51×73×162 mm, represented by a combination of predominantly fatty and soft tissue components, the latter actively accumulated the contrast agent. Multiple vessels ‒ arteries and veins were also traced in the structure of the formation, the latter having a diameter of up to 5 mm. The capsule was not clearly differentiated. The radiologist’s conclusion: «First of all, liposarcoma of the inguinal-femoral region on the right should be considered». Based on the results of the biopsy performed under ultrasound guidance, no data on the tumor process were obtained, signs of fatty necrosis were noted. In a planned manner, radical removal of the formation was performed. After morphological and immunohistochemical examination, the final diagnosis of «hibernoma» was made.

Results: In the presented clinical case, the hibernoma had one of the typical localizations ‒ the inguinal-femoral region. The difficulty of differential diagnosis was due to the presence of a vascularized soft tissue component in the structure of the formation and the absence of a capsule, which may indicate a malignant process. On the other hand, a fairly developed vascular network was traced in the thickness of the tumor, which, according to the literature, can occur with hibernoma and is uncharacteristic for liposarcoma, as it is a low-vascularized tumor. As a result, the diagnosis of hibernoma was confirmed.

Conclusions: A clinical case of a rare fat-containing tumor, hibernoma, is presented. A differential diagnostic sign of liposarcoma may be the presence of a pronounced vascular network in the structure.
